Box #1 | Enter the date you are filling out the form. |
Box #2 | Enter your name and your business name if applicable. This is the name of who the mail will be sent to and can also be your doing business name (sometimes called a fictitious business name) if registered depending on which state you live. |
Box #3 | Enter the following for box 3 options: 3a) C/O Postmarked, 20619 Torrence Chapel Rd Ste 116 #XXXX 3b) Cornelius 3c) NC 3d) 28031 XXXX = your mailbox number |

Box #5 | Enter yes and sign your name as it appears in Box 2. Use just your personal name not your business name. |
Box #6 | Enter your legal name as it appears on your photo ID. |
Box #7 | For boxes 7a through 7e enter your personal home address information as applicable. |
Box #8 | Two forms of identification are required in this section and a reminder, one must be a picture ID. The agent must write in identifying information. Subject to verification. Note: Under box 8b you can see the list of acceptable IDs |
Box #9 | If you are using your mailbox for business purposes enter the legal name of your business |
Box #10 | If you are using your mailbox for business purposes, and your business has a different physical address than what you entered in boxes 7a through 7d, enter your business physical address. If you don't have a physical address for your business you can enter your home address again or leave boxes 10a through 10e blank. |
Box #11 | Enter what your business does and type of product or service you manage and/or sell. |
Box #12 | If you are using your mailbox for business purposes, and your mailbox will be used by multiple people at your company or firm, enter each name that may receive items to your mailbox. The same rules apply for identity so each member needs to submit verifiable identification. Additionally, your plan may limit the number of recipients or charge additional monthly fees for extra recipients. Please see your plan documentation for further details. If you are not using your mailbox for business purposes leave this field blank. |
Box #13 | This is for any member of a corporation who would receive mail at this address. It is even more stringent than a “firm” as you also need to add the address of each of the officers. |
Box #14 | If your business name is registered they actually need the country, state and date of registration. You may need to reference your DBA or doing business as files, also called a fictitious business name in some states. |
Box #15 | This is for the notary or authorized agent to sign. There is nothing for you to enter in this box. In some jurisdictions the notary will sign this box and stamp this form. In other jurisdictions you may have a second sheet for the notary to attach. |
Box #16 | This is your signature to finalize. And if the applicant is a corporation or a firm, only an officer can sign (officer must be listed in box 13). They also have to include their title. |
